DVM Affiliate Program

DVM Affiliates are able to participate in direct online retail sales made by their customers or clients.  Affiliates will need to establish a Customer Account before being able to complete the DVM Affiliate registration.  To proceed with the DVM Affiliate Registration, please click the continue button below. You will also need to fill out a W-9 form and fax it to our offices at 817-594-8202.  Once an application for Affiliate status is received there will be a 24 hour period for application approval and account activation (M-F), and a notice will be received once an account is activated.  At that time, Affiliates can logon to their customer accounts using the 'My Account' link followed by the 'Visit My Affiliate Page' link seen at the website.
